The above link will allow the club secretaries to submit their 5M 9 Club Activities Monthly Report via e-mail. The report will be submitted to the Governor, Vice Governor, Zone Chair, and the e-mail address used at the bottom of the form. 5M-9 CLUB ACTIVITIES REPORT HELPFUL HINTS The above link contains useful hints to tidy up the 5M-9 Club Activities Report to make it easier to read and score. MONTHLY MEMBERSHIP, PU-101, AND ANNUAL ACTIVITIES REPORTS The above link will take club secretaries to the Lions International Membership page where you can enter and file on-line your club Monthly Membership Report (MMR), annual PU-101 Club's Officer report, and the Annual Activities report which are due to International and copies to 5M-9. If you have not used this service before, you will need to get your club number and password in order to access the club account. This information was sent to the club treasurer. Follow the directions on the page. Click on the instructions button and receive a printout on how to use the forms. Check with the current Governor to see if your report will be viewed directly by the Governor and therefore no written report needs to be sent to 5M-9. If you will need to copy and mail or use the email option to email a copy of this report to 5M-9. You may email District Governor Linda. Please e-mail Governor Contest Chair Joyce Ramsey a copy of your MMR show she can record this information.
